Home and community based services (HCBS) provide opportunities for Medicaid beneficiaries to receive services
in their own home or community rather than institutions or other isolated settings.
These programs serve a variety of targeted populations groups,
such as people with intellectual or developmental disabilities,
physical disabilities, and/or mental illnesses.

Community Health choice
If you’re 21 or older and have both Medicare and Medicaid,
or receive long-term support through Medicaid because you need help
with everyday personal tasks, you will be covered by CHC.

Waivers we accept

Attendant Care Waiver/Act 150:  Program enables individuals, ages 21-59, with physical disabilities to live in their own homes and communities. Participants have ultimate control of their services, including the right to recruit, hire, train, supervise, pay and, if necessary, terminate the employment of their attendants. Services available include: Community transition (Waiver only), Personal assistance services, and service coordination.

Independence Waiver:  Program provides home and community-based services for persons, ages 21-60, with physical disabilities to allow them to live in the community and remain as independent as possible. Services available include: Adult daily living, Community integration, Personal assistance, and more.

Aging Waiver:  Program provides home and community-based services to eligible persons age 60 or older that are clinically eligible for nursing facility care. Services available include: Adult daily living services, Companionate services, Home care, Home support, Personal care, and more.

PLEASE NOTE: when choosing an agency to provide these services, mention “AFFORDABLE HOME CARE” as your agency of choice.

Our aides provide exceptional assistance to people who are sick, injured, mentally or physically disabled, or the elderly and fragile.

Private pay

Addition to the home and community-based program, Affordable Home Care also provides out of pocket services.

 If you don’t qualify for homecare services or if you would like to receive more hours of care, you may choose Private Pay.

NOTE: $70 will be credited toward your services bill if you decide to receive services from Affordable Home Care.
